Order Form Instructions

To Calculate the Cost Only To Create a Mailable Order Form To Order by Credit Card
  1. Select your program and shipping option.
  2. Fill in the state field.
  3. Select the Calculate Cost option at the bottom of the page.
  4. Press the preview order button

  2. Fill all your personal information.
  3. Select the Full Order Form option at the bottom of the page.
  4. Press the preview order button.
  5. Print out the new page.
  6. Mail it to the address on the order form.

  3. Check the Credit Card Order box and fill in any special instructions.
  4. Select the Full Order Form option at the bottom of the page.
  5. Press the preview order button
  6. Press the button at the bottom of the new page.
  7. You can call us or we will contact you for your credit card information.
